Re-rupture of the Achilles tendon can be increased up to the first 3 months from initial tear. This patient had an initial repair by another surgeon that re-ruptured soon after coming out of the moon boot.
The repair was taken down. The tendon ends were opposed + an additional LARS graft placed over the tendon ends for reinforcement.

Rupture of the Achilles avulsed from the heel, rarer type of injury but still significant, and not to be treated as per a regular tear of the Achilles. In this case the tendon was retracted almost 6.5cm, making it difficult to repair back to its origin.
The Haglunds of the calcaneus was debrided to create a bleeding surface to repair back to. The white plugs are Tetrous plugs, a biologic plug to optimize enthesis healing to bone. An additional Lars ligament was used to augment the repair, and a Speed bridge repair of the tendon back to bone with a double row anchor repair, placed in the Tetrous plugs to encourage growth/healing back to bone.

This patient had a fixed plantarflexion of the foot, and had been walking on her tiptoes without being able to put her heel to the ground for years.
At time of surgery most of the stiffness was actually in her ankle, nonetheless we lengthened her Achilles, and did a triple fusion, with a closing wedge osteotomy performed through the talonavicular and calcaneocuboid joints to get more dorsiflexion.
Still not perfect, but will do for now. When she's fused we will do a 2nd stage total ankle replacement to get more movement from the ankle with a posterior capsular release as well.

You don't always get it as good, but it's always nice when a flatfoot recon comes together as planned. Patient had a tear of the tibialis posterior tendon that I prefer to repair if not too degenerate rather than perform a tendon transfer.

This patient had the worst luck in sustaining not only 1 but simultaneous, bilateral Achilles tendon ruptures.
Quite a rare occurrence, both were also repaired simultaneously through mini incisions to expedite recovery and mobility. Normally I would place these in plasters for the 1st 2 weeks, but to allow mobility, the dclecision was made to allow immediate stand transfers in bilateral Moonboots with large heel wedges/blocks.

As much as I love ankle replacements sometimes there are still certain indications for an ankle fusion. Surprisingly patient satisfaction is equal to an ankle replacement and rate of complications are reduced.
This patient was still quite young, active and large, therefore the decision was made to fuse him or stop hopefully he might have the opportunity to have this taken down and converted to an ankle replacement in future.

A high ankle sprain or syndesmosis injury can be easily missed as a simple ankle sprain, but have consequences further down the track leading to post traumatic arthritis.
This is an arthroscopy of the ankle showing a probe inserted in the syndesmosis. As I twizzle the probe, the fibula is seen moving away from the talus, displaying instability of the syndesmosis. Even 2mm of shift/movement can significantly alter contact stresses/pressure on the cartilage leading to instability and wear.

Repair is done keyhole (Arthro Brostrom) minimising complications. 6 weeks in a Moonboot, weight bearing after 2 weeks.

A low lying peroneal muscle belly can be a genetic source of pain and can be associated with tears of the peroneal tendons, as seen in this patient. In this case the peroneal muscle extended way past the fibula groove down to the level of the peroneal tubercle.
The muscle belly is debrided and the peroneal tendon is repaired.

Bilateral bunion corrections done minimally invasive. Weight bear immediately for 6 weeks and then back in normal shoes. Importance is to recognise the 3D nature of rotation in bunions, which requires correction to prevent recurrence. Much easier to correct minimally invasive than through traditional open techniques.

This patient came not long after a pain free ankle replacement saying his ankle suddenly became swollen and sore. After multiple investigations including scans and samples he was taken to theater and discovered the implant peg had actually broken off. You can see the metallosis in the wound causing loosening of the talar component.
Revised to a stemmed Inbone implant. Hopefully more success this time around

How technology has changed. This patient had a similar issue on his contralateral ankle, and we opted to do a hind foot fusion nail only 2 years ago. He was happy with his mobility but when his right ankle started to go the same way came back asking for similar treatment, but we opted to go for an ankle replacement instead, and he could not have been happier since.

Quite significant hammer toe deformities that required fusion of the PIPJ's and excisional arthroplasties of the MTPJ's for arthritis and subluxed joints.
This patient had previously undergone necrotising fasciitis to her medial calf/ankle, and the skin was extremely scarred, the presumption being the nec fas potentially scarred the tendons/intrinsics of the foot causing the deformities.

Flat foot reconstruction for a tibialis posterior tendon tear. Good restoration of the arch with repair of the tib post tendon tear, which I try to do more often than an FDL tendon transfer. The talonavicular coverage was adequate so I did a calc shift rather than a Evans calcaneal lengthening.

Valgus ankle predominantly driven by the ankle arthritis, hindfoot was neutral. Corrected with an Inbone total ankle replacement. I feel the Inbone construct more solid with the weight distribution through the tibia makes more sense for longevity of the ankle replacement. Time will tell.

Another keyhole bunion correction for your Monday week. A common misconception is that the "bunion" needs to be shaven away, and this is not always the case. The medial eminence looks more prominent because of the deviation of the metatarsal, combined with rotation of the metatarsal head, hence why keyhole bunion correction is beneficial - allowing a 3D correction of the deformity by correcting rotation as well. Something you can't do with open surgery.

Bad trimalleolar ankle fracture that had significant fracture blisters. Although there's evidence that there's minimal risk of operating through blisters I still prefer to wait for them to dry out, so patient was placed in external fixator and regular wound dressings with silver to dry the blisters out quicker.

This patient had an old injury causing a fracture of his midshaft tibia which healed but caused post traumatic ankle arthritis. Fortunately his long leg films didn't show any angulation of his tibial shaft, mainly translation.
We sent him for a Prophecy total ankle plan which confirmed he mainly had a 2cm leg length discrepancy and valgus angulation in the ankle about 6 degrees.
We opted for an Inbone ankle replacement to accommodate the increased stress likely from the leg length discrepancy rather than the translation of the tibia or valgus of the ankle.

Post traumatic arthritis is still the most common cause of arthritis in the ankle. This patient had quite significant stiffness to dorsiflexion and couldn't even go barefoot around the house because she could barely get her heel down. Three months post up and she is walking barefoot around the house for the first time and is very happy with her progress.

This patient developed arthritis after resection of a giant cell tumour many years ago. Decided to go with an Inbone because of the relative unknown effects a fused syndesmosis may have on an ankle, hoping the Inbone stem will distribute more force away from the ankle. Medial malleolus measured 9mm on the plan, so a prophylactic screw was placed to prevent a stress riser.

This patient had an unfortunate history of being involved in a bad car crash resulting in a triple fusion of her hindfoot. She proceeded to have a ankle replacement when she was in her 40's that eventually failed (1st pic).
She then had a pathological fracture of her tibia that required a tibial nail and plate to stabilise a large cyst above her ankle replacement (2nd pic). Her foot was also in cavus causing recurrent fractures of her 5th metatarsal.
I did her revision in 2 stages, where all metal was removed, the cyst bone grafted, the 5th metatarsal fixed and a cement spacer placed (3rd pic).
After 3 months we went back in to remove the spacer and revise her ankle replacement.

A fracture of the talus that has undergone percutaneous fixation to provide stability. The talus has quite a fragile blood supply, so taking these apart requires careful consideration. In this case the fracture was already 2 months old so we made the decision to help it to heal by providing some stability with the screws.

Have a ganglion Easter! A ganglion can arise from a joint or from a sheath of a tendon where fluid can escape and form in this case, quite a large lump!
Ganglion can be aspirated by radiology, but unfortunately can sometimes recur, in which case excising it works well.

Another severe bunion, combined with arthritis of the midfoot, undergone a Lapidus (fusion of the 1st tarsometatarsal joint) and fusion of the 2nd & 3rd tarsometatarsal joint for the arthritis.
Patient already had the other side done and was happy with result.

This is what a torn peroneal tendon looks like before and after repair.
This can cause pain on the lateral aspect of your ankle. Beware the unstable ankle that can cause overload of the peroneals eventually leading to a tear.
In this case we were able to tubularise/repair the tendon with sutures, in some cases a tendon graft or tendon tenodesis may be required.

Total ankle replacements are little known to the general population. Ankle arthritis is much rarer than hip or knee arthritis, cartilage in the ankle is much thinner, yet surprisingly more robust.
Regardless an ankle replacement can preserve motion, a much better alternative than having your ankle fused.

Hallux varus occurs most commonly when bunions are overcorrected. This leaves a large unsightly gap between the toes and also causes the toe to cock up. This lady had rubbing on shoes and couldn't dance.
The picture below shows the correction by reconstructing the lateral collateral ligament of the joint 6 weeks post op.
